Without help, many adolescents become suicidal through puberty!
Born in the wrong body - or if the biological gender does not match your own sense of identity. In the past, this was mostly understood as a psychological problem that should be treated - today, the trans identity is taken seriously, explains chief physician Dagmar Pauli.
If the biological and emotional sex do not match, one speaks of a transgender identity. This often becomes an issue during puberty. Difficult time for such a radical diagnosis?
Dagmar Pauli: Puberty is a difficult phase for all young people. There are so many changes, mental and physical, with the development of sexuality. For trans young people, this development creates even more stress - for a feeling of being a stranger in their own body. There is an early trans-identity that emerges in childhood - but there are also many cases where the person first expresses the foreignness during puberty or later, or only becomes aware of it later. You can compare that with homosexuality - even those who came out late are no less “genuinely” homosexual.
Why not just wait?
Puberty creates enormous suffering for trans young people. Many become suicidal during this time if they don't get help. In addition, puberty shapes the outward appearance for later life - if this does not match the psychological gender, this severely limits the quality of life in the long term.
According to studies, there are one to three percent of people who later regret having sex reassignment.
Subsequent interventions are more difficult. That is why there is the possibility of delaying the changes - for example a broken voice or the growth of the breasts - with so-called puberty blockers in order to gain time.
Can minors decide on treatment themselves, if necessary without the consent of their parents?
Basically it is possible. It depends on whether the minors are capable of judgment - this is what the treating doctors assess. The ability to make judgments about medical treatments is not based on age, but depends on mental maturity and the topic. Whether or not ingesting hormones is the right way for a person is a complex question.
Support from their environment is very important for the young people.
Mature minors are able to make judgments about this, then they can legally make the decision without the consent of their parents. In all cases, however, the parents are involved in the decision. Your perspective is important, but you need to be open and inquiring to understand your children's suffering. Support from their environment is very important for the young people.
Hormones are also administered for gender reassignment. These intervene strongly in biology.
Yes. Thanks to the puberty blocker, you can wait with the gender reassignment hormones. The aim is to be able to say with a probability bordering on certainty that the person has a permanent transgender identity before the hormone is released. According to studies, one to three percent of people later regret their gender reassignment. Puberty blockers are reversible. Even those who have taken hormones and discontinue them can still have children - but there is no guarantee. Some people have their sperm or eggs frozen. Taking hormones is not a decision to be taken lightly. The affected person must also be informed about side effects, such as an increased risk of thrombosis.
